Terms of Consent - Halton Region, Ontario

Please review these terms of consent for participating in our SmartParent text-message prenatal and parenting education program.

By enrolling in SmartParent you:

  1. Agree and consent to receive text messages from SmartParent.
  2. Acknowledge the right to discontinue your enrollment in SmartParent at any time without giving a reason and without any negative impact on you. You can discontinue receiving text messages from SmartParent by texting “STOP" to the sender (12323) or by cancelling via the website portal.
  3. Accept that SmartParent will not provide you with any personalized medical advice with regard to your or your child’s individual circumstances and/or your or your child’s medical condition.
  4. Accept that, although SmartParent will not charge you to sign up or to receive the text messages, standard message and data rates may apply. This means that you may be charged by your mobile phone service provider (e.g., Bell, Rogers, Telus, etc.) for receiving/sending SmartParent text messages and/or browsing links included in the text messages, depending on the type of phone plan you are currently on.
  5. Acknowledge that only SmartParent and technology affiliates will have access to your data.


Personal Information

Personal information is information we hold which is identifiable as being about you.

Our collection, use, and disclosure of your personal information is regulated by the Personal Information Protection and Electronic Documents Act (Canada) generally, and the Personal Health Information Protection Act (Ontario), as amended from time to time. You can find more information about your privacy rights at the Office of the Privacy Commissioner of Canada (OPC) and the Information and Privacy Commissioner of Ontario (IPC/O). 

We collect information from you when you enroll in the program including:

  • Your cell phone number
  • Pregnancy due date
  • Your baby’s birth date
  • First 3 digits of your postal code

Your data are held by SmartParent. Data are stored on secure servers located in Canada, in password-protected files. Data such as your cell number or birth date will be shared with your local health unit for program planning and data analysis purposes. Your other personal identifiers will only be shared with your local health unit on an as needed basis to provide services.

Security

While no service is completely secure, we take all reasonable measures to protect information about you against unauthorized access, use, alteration, or destruction.

Requesting access or correcting your personal information

Subject to applicable local laws and regulations, you may have some or all the following rights with respect to your personal data:

  • to request access to your personal data and to rectify any inaccuracies within that personal data;
  • to withdraw your consent to our processing of your personal data.

If you wish to contact us to with a request relating to personal information we hold about you, please contact us using the contact details set out below including your name and contact details. We may need to verify your identity before providing you with your personal information. We will deal with all requests for access to personal information within a reasonable timeframe.
